Privacy Policy
What this website collects, why, and who it goes to.
The short version
- When you book, this site asks for your name, email and phone, plus two optional questions.
- We use that to schedule your call and talk with you about your care. We don’t sell it, and we don’t use it for advertising.
- The site has no analytics, advertising pixels or tracking cookies.
- Your medical record lives in our patient portal, Charm Health, not on this website.
Who we are
Portal Wellness is a physician-led telehealth practice for patients located in North Carolina. In this policy, “Portal,” “we” and “us” mean Portal Wellness.
This policy covers this website, joinportalwellness.com, and the booking form on it. Once you become a patient, your medical information is kept in your patient record in Charm Health and handled under the privacy rules that apply to medical records.
What we collect
When you book an intake call
- Your name, email and phone number
- Whether you’ll be in North Carolina for your visit
- The time you choose for your call
- If you choose to answer them: your gender and which treatments you’re interested in
If you’re outside North Carolina
If you ask us to let you know when we can see you, we collect your name and email, and your phone number if you gave one.
Automatically
Whenever a website loads, the server receives basic technical information: your IP address, browser type, the page requested and the time. Our hosting provider processes this to deliver the site and protect it from abuse.
What we don’t collect
No analytics, no advertising pixels and no tracking cookies. This website doesn’t ask for your medical history, medications or lab results. Those come later, inside the patient portal.
How we use it
- To schedule your intake call and send your confirmation
- To contact you about your booking and your care
- If you’re outside North Carolina, to tell you when we’re licensed in your state. That’s the only thing we use it for.
- To keep the site running and secure
We don’t sell your information. We don’t share it with advertisers. We don’t use it to build a profile of you.
Who it goes to
A few companies run parts of this site for us. They get only what they need to do that job.
- Vercel hosts the website.
- Cal.com runs the scheduling calendar on the Book page. Your name, email, phone number and any optional answers you give go to Cal.com with your booking.
- Google stores out-of-state notification requests, through Google Forms.
- Charm Health is our patient portal and medical records system, once you’re a patient.
Each of these companies handles information under its own terms and privacy policy. We may also disclose information when the law requires it, to protect someone’s safety, or as part of a sale or reorganization of the practice.
The optional questions on the booking form
Gender and treatment interests are optional. If you answer them, they go to Cal.com with your booking. Cal.com is a scheduling service. It isn’t part of your medical record and isn’t held to the same privacy rules as your medical record.
If you’d rather not share those details there, leave them blank. You can tell us on your call, or in the patient portal once you have an account.
Cookies
This site doesn’t use cookies for analytics or advertising. The scheduling calendar on the Book page is provided by Cal.com and may set cookies it needs to work. The patient portal is a separate site run by Charm Health, with its own policies.
How long we keep it
We keep booking details for as long as we need them to schedule and follow up with you, and longer only where record-keeping rules require it. We keep out-of-state requests until we’ve told you we’re licensed in your state, or until you ask us to remove you.
Your choices
Email info@joinportalwellness.com to see, correct or delete what you’ve given us through this website, or to come off the out-of-state list. If you’re a patient, some records have to be kept by law even if you ask us to delete them. When that’s the case, we’ll tell you.
Security
We use established providers and limit who can see your information. No system is perfectly secure, and we won’t claim otherwise. If you think something has gone wrong, email us.
Age
Portal treats adults. This site isn’t meant for anyone under 18, and we don’t knowingly collect information from anyone under 18.
Changes to this policy
If we change this policy, we’ll update the date at the top of this page. If a change affects how we use information you’ve already given us, we’ll say so plainly here.
