The rules for using this website. These are separate from the agreements you’ll sign if you become a patient.
The short version
- This site is information, not medical advice.
- Using it, or booking an intake call, doesn’t make you a patient.
- We can only treat you while you’re physically in North Carolina.
- Portal doesn’t provide emergency care. If it’s an emergency, call 911.
Agreeing to these terms
These terms apply to joinportalwellness.com, run by Portal Wellness (“Portal,” “we,” “us”). By using the site, you agree to them. If you don’t agree, please don’t use the site.
Not medical advice
Information on this site is general. It isn’t medical advice for your situation. Whether any treatment is appropriate for you is a decision made with a physician after an evaluation.
When you become a patient
Browsing this site or booking an intake call doesn’t create a physician-patient relationship. That begins only after a Portal physician has evaluated you and agreed to provide your care. At that point, separate patient agreements and consents apply.
Emergencies
Portal doesn’t provide emergency care. If you’re having a medical emergency, call 911 or go to the nearest emergency room.
Who can use our services
- You must be 18 or older.
- You must be physically located in North Carolina at the time of each visit. Our physician is licensed in North Carolina, and that’s where we can treat you.
- The information you give us must be accurate and about you. Please don’t book on someone else’s behalf.
Services, prices and results
The services and prices on this site describe what we offer today, and they may change. The fees and payment terms that apply to you, including cancellations and refunds, are given to you at enrollment, before you pay.
Medical care doesn’t come with guaranteed results. We don’t promise any specific outcome, and nothing on this site should be read as one.
Labs, pharmacies and other providers
Lab testing and pharmacy services are provided by separate companies, which have their own terms and charges. Lab work is billed separately, and we’ll tell you what it costs before anything is ordered.
Links to other sites
This site links to other websites, including our patient portal (Charm Health), our scheduling calendar (Cal.com) and public records such as OpenPowerlifting. We don’t control those sites, and their own terms and policies apply.
Our content
The text, design, logo and images on this site belong to Portal Wellness or are used with permission. You’re welcome to read, print and share pages for personal use. Please don’t copy or reuse them commercially without asking.
Using the site properly
Please don’t try to disrupt the site, access parts of it you’re not meant to, collect data from it by automated means, or use it for anything unlawful.
Disclaimers
We work to keep this site accurate and available, but it’s provided “as is.” We can’t guarantee it will always be error-free or online.
Limits on liability
To the extent the law allows, Portal isn’t liable for indirect or consequential losses that come from using this website. This doesn’t limit any responsibility we have for medical care we provide, which is governed by your patient agreements and by law.
Governing law
These terms are governed by the laws of North Carolina.
Changes to these terms
If we change these terms, we’ll update the date at the top of this page. Continuing to use the site after a change means you accept the updated terms.
